  • WHERE IS YOUR OFFICE LOCATED?

  • HOW DO I OBTAIN A NOTARY COMMISSION

    Contact PSI Services (the national testing vendor for the state) at 1-800-620-5802 to begin the commission process. We suggest you prepare for the test by taking a notary training workshop from one of the vendors listed on our website .

  • I HAVE CHANGED MY ADDRESS, WHAT DO I HAVE TO DO?

    You have 30 days in which to notify our office of the change. Obtain a “Bond Policy Rider” from the insurance agency that wrote your notary bond for you. The rider will reflect “old address out, new address in”. Send the rider, your old notary certificate and a $ 5 fee payable to the State of Utah . * Make sure you destroy your old Notary Seal – It is no longer valid.

  • I HAVE CHANGED MY NAME, WHAT DO I HAVE TO DO?

    The same as above, but also include a copy of the official document that changes your name.

  • HOW DO I RENEW MY NOTARY COMMISSION?

    Contact PSI Services at 1-800-620-5802 which begins the commission process. PSI charges a fee of $30 to review each test and issue the test result score. This fee is NOT waived for State employees.

  • HOW DO I OBTAIN A NOTARY COMMISSION?

    Same as above.

  • I SENT IN ALL OF MY NOTARY PAPERS. WHAT IS THE STATUS AND WHEN WILL I RECEIVE MY NOTARY CERTIFICATE?

    Call the Notary Office at 801 538-1041 to verify your status.

  • WHERE DO I GO TO OBTAIN MY NOTARY SEAL?

    Look in the yellow pages under “Rubber Stamps”. For fast service select a local rubber stamp company that manufactures their own stamps. It is best to avoid “office supply vendors” since they send out to third parties out of state who are not familiar with Utah notary law requirements for notary seals. * The State cannot recommend any particular vendor.

  • HOW DO I ORDER A NOTARY BOND?

    Look in the yellow pages under “BONDS” or “INSURANCE”. Contact an insurance company that issues bonds. Most notary bonds cost $50.

  • I AM A STATE EMPLOYEE. HOW DO I OBTAIN A NOTARY BOND?

    Call the Notary Office at 801 538-1041, request a State of Utah Risk Management Notary Bond Form for State Employees.

  • HOW DO I HAVE A NOTARIZATION CERTIFIED OR AUTHENTICATED?

    The Lieutenant Governor's Office authenticates documents that have been notarized in Utah and will be filed in a foreign country. They authenticate and attach an apostille or certificate.

  • DO I HAVE TO PAY ANOTHER $30 PROCESSING FEE IF 30 DAYS HAS EXPIRED?

    If the Notary application and notary bond is returned to the applicant because of deficiency, an additional $30 processing fee may be assessed if the paperwork is not returned within 30 days.

  • WHEN WILL THE STATE SEND ME MY NOTARY STAMP?

    The State does not issue notary stamps. The State issues Notary Commission Certificates. The certificate must be taken to a rubber stamp company as proof of commission. The certificate authorizes the vendor to take the order for a notary stamp.

  • HOW DO I FIND A NOTARY IN MY AREA?

    The quickest way to find a notary in your area is at your local area financial institution.

  • I HAVE SUBMITTED ALL OF THE APPROPRIATE PAPERWORK AND FEE BUT HAVE NOT HEARD ANYTHING. WHEN WILL YOU SEND ME MY APPROVAL OR REJECTION?

    After an application is approved, a notarial certificate of authority will be issued within 3-5 business days. If an application is denied for a deficiency, it will be returned within 3-5 business days.

  • I DIDN'T WITNESS A SIGNATURE BUT I NOTARIZED IT ANYWAY BECAUSE THE SIGNATURE WAS FAMILIAR TO ME.

    ALL notarizations require “personal appearance before a notary”. If a notary signs a notarial certificate without personal appearance of the signer, this would be considered official misconduct and grounds for revocation or suspension of a notary commission.

  • WHEN I HAVE MY NOTARY BOND NOTARIZED ON THE BACK, IT ASKS FOR PAGE NUMBERS. DO NOTARY BONDS HAVE PAGE NUMBERS?

    The page number referred to is the page number of the notarial journal where this notarization is recorded. This question refers to the information located below the oath of office on bonds issued by Western Surety Company.

  • DO I PUT MY CURRENT COMMISSION DATE ON MY BOND OR MY ORIGINAL WHEN RENEWING?

    The notary does not write in commission dates on their bonds. The date is issued by the insurance agent/or the notary office.

  • WHY WON'T PSI IDENTIFY THE QUESTIONS THAT ARE MISSED ON THE NOTARY TEST?

The test administered by PSI is a competency test . It is not the same as taking an educational test. It is expected that the applicant can pass this minimal competency test having minimal knowledge of their responsibilities as a notary.

The purpose of the examination is to determine if those seeking a commission have read and understood Utah law relating to the duties and obligations of a notary. This is for the protection of the notary, as well as for the public who relies on the competency, professional conduct and expertise provided by a notary.

  • DO ALL PARTIES, WHO ARE LISTED ON A DOCUMENT, NEED TO BE PRESENT BEFORE IT IS NOTARIZED?

    If a document has multiple signers and all signers cannot appear together, the document will require one notarial certificate per signer. At the time of each notarization of each signer, a notary will complete a notarial certificate for that signer and staple the certificate to the document.

  • IS NOTARY INFORMATION AVAILABLE ON THE WEB?

    YES. See our Notary website.

  • DOES THE OATH OF OFFICE ON THE BOND HAVE TO BE SIGNED AND NOTARIZED BEFORE IT IS SENT TO THE NOTARY OFFICE?

    YES. Make sure to sign your oath of office in the presence of a notary who will administer your oath of office and notarize your signature on your bond.

  • CAN WE FILE COPIES OF OUR DOCUMENTS OR DO YOU NEED THE ORIGINALS?

    We require the original documents be filed in our office.

  • WHAT IF MY PAPERWORK IS LOST IN THE MAIL?

    Contact your insurance agent and request a duplicate original bond. If you kept a copy of your application we will accept that, if not you can obtain a new application from our website. If you kept a copy of your notary test we will accept that. If you did not, contact PSI for a copy. PSI charges a fee for a duplicate.
