Change of Name and Address

46-1-20 Change of Name

Within 30 days after the change of the notary’s name, the notary shall provide to the Office of the Lieutenant Governor a bond policy rider.

To obtain a bond policy rider, the notary shall:

Notify the surety for the notary’s bond;

Obtain a bond policy rider reflecting both the old and the new name of the notary;

Return a bond policy rider, the original “Certificate of Authority of Notary Public”;

If a name change, return a copy of the official document changing the name;

Pay a $5 fee; and

Destroy the old official seal.

46-1-20 Change of Address

Within 30 days of a change in the notary’s address, the notary shall provide the notary’s new address to the Lieutenant Governor.

A new notary certificate will be issued to the applicant
